- Notification of designated contacts of subscriber location and condition in the case of a medical emergency
- Follow up call to primary physician after visit to emergency care facility
- Follow up call to subscriber after release from healthcare facility to assist with monitoring of medical condition
In addition, all subscribers receive the following accessories to ensure that the subscriber knows how to access their information or to allow healthcare professionals to access the information in the event that the subscriber is incapacitated:
- Emergency medical triage ID card
- Key chain ID
- Window stickers for the home or auto
Standard Pricing: standard service is provided for a cost of $100/yearly (Sign up now and recieve discount of $50.00) subscription + $50 one time set-up fee. Standard service includes one page of medical emergency information at no additional cost.
Optional services and fees include the following:
- Services are also available for customers traveling outside of North America. Phone calls and faxes to parties outside of North America are billed to the subscriber at an additional cost of $1/minute.
- Additional pages of medical information can be captured for an annual cost of $5/page.
- Additional calls to other physicians designated by the subscriber for cost of $1/call.
- Emergency medical triage information on mini-disk (cost: $5)
- Availability of advance directives provided for an annual cost of $15/year.
